GENERAL
Please read through the instructions before completing the application. To complete the application, you should type information directly into the PDF forms provided (parts A – I). The Supporting Materials (parts J-N) should be labeled and attached in order using plain, recycled 81⁄2” x 11” white paper. Create seven double-sided hard copies of the printed application, 3-hole punched, using a paper clip to collate each copy.
Please be sure to review your application and supporting materials for completeness prior to submission. Applications that are missing required information or supporting materials will be considered incomplete and will not be reviewed by the MGP panel.

Applications must be postmarked no later than Wednesday February 15, 2012. Please use USPS First-Class or Priority Mail. (Express Delivery, UPS, Fed-Ex, or other costly services are not necessary.) Include a self-addressed stamped envelope (SASE) if you want your CD returned.

THE APPLICATION
A : Cover
Select the appropriate categories for which you are applying. Check off each completed part of the application.
B : Information Page
In addition to the basic information, applicants should include a brief description of the proposed project in the space provided. In 75 words or less, describe the project and the goal of the project.
C : History/Background of applicant (one page) Applicants should include a strong artistic statement about their mission and purpose, background, and history.
D : History/Background of composer, if applicable (one page) An application for an ensemble with a composer must include the composer’s biography and other relevant information.
E : Project description (two pages) The project description should address the points below. Depending on your project, some points may be more applicable and/or may require more description than others.
Project: Describe your project in detail. What will you do? What do you need? Why do you need it?
Motivation: Describe why you have decided to undertake this project. Address the impact the project will have on the group and, if applicable, the local community. Why is this project important and worthwhile?
Promotion and Publicity: Describe the outreach you will do in order to share your project with the public. If you are applying for a project that involves public performance, how will you promote the performance? If your project is intended to better organize or develop the group, how will the proposed improvements eventually be shared with the public?
Budget: If the funds you request will be used in conjunction with other funds as part of a larger project, describe your other sources of funding and how the MGP funds fit into your overall budget.
Timeline: Describe briefly the project timeline and the major dates relevant to your project.
F : Performance History (one page) List the date, venue, and programs of the ensemble’s performances for at least the past two years. If a complete list of performances does not fit on one page, please create a selected list containing those public performances that best support your proposed project and that will be most useful to the selection committee.
G : Project Budget (one page) The project budget should show possible sources of income for the project, as well as projected expenses. Use the included template, entering figures as necessary. You do not need to enter a number for each box. You do need to create a balanced budget i.e. equal amounts for Income and Expenses. Additional lines have been provided in each category for you to use and label as needed.
H : Budget Notes (one page maximum) The budget notes should provide more detailed information about the line amounts in your project budget. All notes should identify and correspond to the line items in the budget. For an example of possible specific line information, click here.
I : Work Sample List
Provide information for the musical samples included in the Work Sample CD. This list should correspond with the tracks on the CD that is submitted as part O of the application.
SUPPORTING MATERIALS
The following supporting materials should be attached, in this order, to the application. There are no forms provided, so please make sure that each part is clearly labeled with the name of the ensemble/organization and the title of the document.
J : Press or PR Materials
Press or PR materials are limited to one item, such as one press review; season brochure or concert program copied on a 3-hole punched 81⁄2” x 11” sheet. (Please do not include loose postcards or other printed materials)
K : General Financial Information
Please provide a copy of a Statement of Income and Expenses (also called a profit and loss statement), for the previous year or performance season, or for the last project performed by the applicant. (See Resources page for templates.)
L : Statement of Financial Position
Applicants with annual budgets over $50,000 are required to provide a copy of a Statement of Financial Position (also called a balance sheet). For applicants with annual budgets under $50,000, this financial report is optional. (See Resources page for templates.)
M : Letter of Commitment
If the application is for a project that is a collaborative effort between an ensemble and composer, between an ensemble and presenter, or between all three, please include a letter of commitment signed by at least one member of each interested party. Please plan ahead, this letter must be submitted with your application.
N : Score
If applying as an ensemble with a composer, please include one score as a sample of the composer’s work. The score can be a selection from a longer composition.
O : Work Sample CD
All applicants—ensembles and presenters—must include one CD.
Please submit two contrasting works/excerpts. Each sample should be not longer than 4 minutes in length, and one sample should preferably be from a live performance. Clearly label your CD with the ensemble/organization name. Make sure you include the Work Sample List with Track information (part I above).
Additionally, if applying in the “Ensemble with a composer” category, one of the two audio work samples must be by the composer and the score should be included. Samples should be chosen carefully to show an applicant’s best work in the time allotted. Sorry, we are able to review CD’s only.
Work Sample Choices: There are several important considerations you should make when choosing your work samples:
Please bear in mind that the panelists listen to many recordings, and really want to get a complete sense of your ensemble’s strengths. The quality of the sound recording is important, as is the variety of music presented. Please do choose contrasting selections. In addition, it is very helpful if your CD is related to the project you are applying for.
If you are a composer, it would be helpful for the panel to hear a type of music (instrumentation or size of ensemble) that relates to the proposed project.
For presenters, we request that you compile one single CD with selections from the groups that are proposed in the application, sticking to the 8 minute maximum total duration. We cannot accept multiple commercial CDs.
P : California Cultural Data Project Report or Tax Return
For applicants with annual budgets above $50,000, a California Cultural Data Project (CDP) report or a tax return covering the same period as your financial year is required. Applicants submitting a CDP may use the same report format generated for the San Francisco Hotel Tax Fund/Grants for the Arts. To obtain your CDP report visit the California CDP website.
